My read on it was that boy-girl is a butch lesbian and/or gender nonconforming, and Xiao Xuan mistook her for a boy and crushed on her, but either was straight or didn't dare question her sexuality on top of all her other burdens.

Since she did his fitness for police work assessment, I assume the psychologist works for the police department in this capacity in general, so procedure about confidentiality might be different.
